Provide the following information to the High School Registrar:
Proof of Residence
(Only a Lease, Contract, Deed, or Closing Statement can be accepted.
If an Apartment Lease is presented, the student must be listed as one of
the occupants.)
Final Report Card
(1st - 9th Graders) or Transcripts(all
other students)
Student cannot be enrolled until this information is provided.
Immunization Records
Texas law
requires all students to be immunized for certain reasons.
According to school policy, children may be exempted from the
requirements if parent/guardian furnishes certification from a physician
that one or more of the immunizations would endanger the child's life;
or submit a signed statement that immunizations are contrary to their
religious beliefs.
Immunization
will be required for the following diseases:
DT
A series completion of
DTP or DT, plus a booster dose after the 4th birthday and within the
last ten (10) years.
POLIO
At least three or more
doses or oral vaccine, one of which must be after the 4th birthday.
RUBEOLA
(Hard Measles) One
immunization or disease on or after 1st birthday and after January
1, 1968. (Disease must be verified by physician's statement.)
MUMPS
One immunization or
disease through age 19. (Disease must be verified by
physician's statement.)
Birth Certificate
(there is a 30-day grace period on this item)
