8:00 a.m. - 11:00 a.m. and 12:00 p.m. - 2:00 p.m.

Fill in the form below with three different choices for requested times.  Please do not fill in multiple forms as they will be moved to the bottom of the request list.  There are 35 available appointments within each hour of registration. Please make an appointment even if your child cannot attend with you.

Due to traffic considerations,
please plan to arrive just prior to your appointment hour.

An email will be sent to the site administrator at the time of submission.  You will need to check the calendar below to verify the time you receive.   The site administrator will be posting the times.  

There is no guarantee that your three choices are accepted due to the large number of people registering.
