Required Paperwork

The following forms must be on file before an athlete will be allowed to practice or participate in a contest.  Click on the links below.

• Parent Volunteer Survey (Track and Field only)

• Acknowledgement of Policies

All athletes who are registered in the athletics classes for the upcoming school year will need to have a physical all the above paperwork completed and on file with the coaches prior to the issuing of equipment and trying out for athletic teams. Paperwork may be obtained from the coaches offices, downloaded from the Paperwork section on the Boys Athletics web site, or downloaded from the RRISD web site.
